It was designed on both sides by James Butler, who is best known as the designer of the Queen's Great Seal of the Realm. This coin didn't use any of the official portraits of the Queen, instead featuring Butler's take on Her Majesty on the obverse and a depiction of the St. Edward's Crown, worn for coronations, on the reverse.
